Primary Care Membership Options
New patients seeking primary care services are invited to join a membership plan that best fits the needs of their family. For specific concerns I create a tailored 3 to 6 month program (see the “Specific Concern” page for more information). For families that need or prefer more than occasional well child or sick visits, or for families dealing with chronic conditions, I have a membership tier called “Premium.” It is a comprehensive, concierge membership plan that includes unlimited visits, 7 days a week direct access to me, and an assortment of labs, tests, and services.
For families who want a consistent connection with me while maintaining a simpler care structure, I have a membership tier called “Essentials,” which includes a yearly comprehensive visit and direct access to me through the portal during business hours. Labs, tests, and additional services are billed separately.

You can find my current fee schedule here. If you sign up for membership at your initial visit, your first month of membership is complimentary. I will save your card on file during check out and set up auto-pay for your monthly payments.

You may cancel the membership at any time with 30 days notice by contacting the office. If you cancel before 12 months and services used are more than membership dues paid you will owe the difference. We will charge the difference to the card on file. Please let my office admin know if you need to make other arrangements.

I accept payment for memberships by credit card, HSA card, and FSA card. I do not accept insurance. Monthly membership payments are automatically charged to the card on file.

Step 1: Please fill out this formor contact my office administrator to have your child added to our patient portal called Charm.
Step 2: Check your email for a PHR invitation from Charm to register a Patient Portal account. If you do not see it in your inbox, please check your spam folder. Once you have access to the patient portal you can request an appointment. Julia will then confirm the appointment time you requested. In some instances she may ask you to pick another time or adjust the time you requested a little earlier or later based on the schedule that day. If you do not see a day or time that suits your needs please message Julia on the portal and she will do her best to find a time that works for you.
Step 3: After your appointment has been scheduled, you will be sent new patient intake forms. I recommend setting aside at least 30 minutes to fill out the new patient intake forms and if you do have to leave the form before fully completing it, make sure to save your progress (at the bottom of the page.) I also recommend you do this on a computer as some patients have had difficulty with information not saving if done on a mobile device. Please have these forms completed 24 hours prior to your appointment so Julia can review them.

We use Charm EHR for our patient portal. Once registered, you will have the ability to securely message Julia, access current data, such as medications and labs, and access treatment plans.

Our office is in Wichita at The Pause which is at 2909 West 13th Street North.
It is a gray building at the southwest corner of 13th and St. Paul. The door to the lobby is open and I try to be at the front desk to receive you when you arrive. If I am not at the front desk, you can wait in the lobby and I will be with you shortly (currently there is no receptionist). Feel free to help yourself to tea!

For questions that are time sensitive, like last minute appointment changes or questions about your appointment time or location, you can TEXT Julia at 316-303-2664. Premium members may also text Julia at this number after hours until 9:00 pm and on weekends.
